The mission of the program is to provide services that assist individuals with mental health diagnosis integrating into the community, improving quality of life, and enhancing wellness, empowerment and recovery efforts. We adapted a holistic one stop approach offering a myriad of services includes: processing HRA supported housing applications, assistance with entitlements, group services, escorting, Wellness and Recovery practice, resources and information.

Baltic Street Resource and Wellness Center
The Center promotes community inclusion through the use of natural supports and normalized roles. The Center is a place where a person can learn to balance the mental, physical and spiritual aspects of their lives. This is accomplished through traditional peer centered tools such as the 8 Dimensions of Wellness, WRAP, Harm Reduction, and healthier eating and budgeting and introducing other person-centered wellness concepts to the Center’s participants. The Center also focuses on alternative healing practices through art, yoga, music, writing and philosophy workshops. As well as several Clubs including Book, Poetry, Photography and Gardening. Each Peer Resource team member brings their own individual gifts and talents to the groups, workshops and clubs that they facilitate.

Staten Island Bridger II
Bridger ll follows the holistic approach of the 8 Dimensions of Wellness by motivating and encouraging participants to adopt and foster habits of wellness. Bridger ll works closely with the treatment team of South Beach Psychiatric Center (Case Managers, Social Workers, Nurses, sychiatrists). Once the participants are discharged to the TLRs (Transitional Living Residences) at the site, the Peer Bridger continues working with the participants by providing support to help them achieve their goals and live a healthier life in the community. Eligibility: Adults who are in the discharge process from South Beach Psychiatric Center.

Staten Island Bridger III
Bridger III follows the 8 Dimensions of Wellness and encourage all participants to foster habits of wellness. Our participants are from the Transitional Living Residences (TLRs) on South Beach Psychiatric Center, while they prepare for discharge. Bridgers work closely with their participant’s and their treatment team (i.e. Case Managers, Nurses, Social Workers, Psychiatrists, Discharge Planning) to discuss goals to provide support for each client.
Eligibility: Adults enrolled in South Beach Programs.

HomeWORKS is a supportive housing program which is operated and managed by Baltic Street Advocacy, Employment and Housing Incorporated, which is the largest consumer-run agency in New York State. We provide housing for persons living with an Axis I diagnosis in scattered-site housing. Our members/tenants reside in the boroughs of the Bronx and Brooklyn. Eligibility: Adults 18 years and older with a mental health diagnosis.

Community Links Education Services
Community Links is a mobile, community-based Supported Education (SED) service which assists individuals living with mental health conditions across NYC in pursuing education goals and successfully completing TASC (GED), college, certification, licensing, or ocational degrees. Since May of 2015, Community Links has supported participants in realizing their full potential through educational achievements which promote independence and improve long-term work opportunities.

Baltic Works is the new employment program of Baltic Street AEH, Inc. Baltic Works is an employment network that accepts Ticket-To-Work from Supplemental Security Income (SSI) and Social Security Disability Income (SSDI) recipients.Our mission is to provide stellar employment services to Baltic Street participants. The Geriatric Advocacy Program is here to assist and serve individuals aged 60 and over with mental health lived experience throughout the five boroughs.
The purpose of the program is to help individuals attain/sustain a well-balanced standard of living, with a holistic approach that includes: The 8 Dimensions of Wellness, Wellness Recovery Action Plan (WRAP) and other evidence-based models with the inclusion of peer supports along the path of growth and success.

Baltic Street AEH Inc. , Adult Home Initiative is here to serve you as part of the class action lawsuit, United States of America Vs. New York State (2013). Baltic Street Peer Bridgers are tasked with assisting the fellow class members to identify & achieve their personal goals. Educate the class members around an array of different services that will allow them to be able to make an informed decision a successful integration back into the community. Baltic Street will work with you to achieve your goals utilizing a person-centered approach, the 8 dimensions of wellness and the WRAP plan.

B.H.O.P.E.
Baltic Street, AEH partnered with Community Care of Brooklyn, a network of medical and social service providers throughout Brooklyn. The B.H.O.P.E Program began in March of 2018. Our peers work at Maimonides and Interfaith Medical Center where they connect with patients who are admitted onto the Inpatient Units. Peers work collaboratively with the inpatient teams and care managers by attending rounds, community, family, and discharge meetings. Peers develop a trusting relationship by building rapport with patients that foster trust and hope for positive change, choice, wellness, recover and personal empowerment. Eligibility: 18 years or older, client of Maimonides and Interfaith Inpatient unit with mental health history.
